Welcome and thank you for taking a few minutes to complete some necessary paperwork prior to your first appointment. The New Client Welcome Packet is provided here in digital form (pdf). Please print the paperwork and mail to the office address provided below before we make a first appointment or bring it with you to your first in-person meeting.
– You will find a Welcome Letter from Dr. Armbrust, the CCC, and the HIPAA Privacy Practice Notice. These are for your information and there is no need to return them. Welcome Letters & HIPAA Notice
– Please complete, to the best of your ability, a Client Registration and Personal History form for each person participating in counseling. Please note that there is an Adult form and a Child and Adolescent form. Please choose the correct form for each person. Adult History & Registration Child & Adolescent History & Registration
– Next the Financial Agreement is a document that clarifies payment. Only complete the self-pay portion or the insurance portion depending on what method you plan to use. In either case, please be sure to sign this form. The accompanying late cancel and missed appointment form asks that you sign and check a box indicating that you understand the policy on this. It also provides a way for you to supply credit, debit, or HSA card information and permission to keep your card on file for charges such as session copays, coinsurance, late or missed appointment fees, and self-pay payment if you desire. – Finally, please complete the Informed Consent form. Each participant (except children under age 14) should initial each bottom corner and sign the last page. Note that there is one additional page at the end specifically for Telepsychology that requires a signature as well. Informed Consent with Telehealth
